To Register or for More Information:
Please contact Paige at 303.429.2100 or email at firstname.lastname@example.org. She will email you a packet of information and forms. You will be required to sign a Disclosure Statement, Confidentiality Statement and complete an authorization form for use of your credit card. Once these forms have been received via email or FAX, a confirmation will be immediately sent giving you further instructions on how to participate in a group. It is recommended that you make a commitment to consistently attend one particular group rather than drop in sporadically. This way, a continuity of topics discussed and cases presented can be maintained.
